Re-Authorization Form

Re-authorization is offered free of charge to licensed software holders with a valid approved reason three times. For additional information, review Authorization and Re-Authorizations. Reauthorization cannot be approved unless your software license is valid and current.

Please follow the steps below to re-authorize your software.

Enter the site code that is displayed on your screen on the following form.

Execute registration program by entering REGISTER.EXE at DOS prompt while in C:\BE or c:\BX directory. Enter company name and serial number on screen, hit return. Enter the displayed registration key on the following form.

On-Tap Products

Execute ONTAP_REGISTER.EXE. Fill in and obtain information as above. Enter the registration key on the following form.

Execute PW_REGISTER.EXE. Fill in and obtain information as above. Enter the registration key on the following form.

By completing the Re-authorization form I certify that the statements are true and that the authorization code I am receiving is not being used to make a functional copy of any ISD software application(s) other than the one which was disabled due to the above reason(s). I certify that if the reason for the authorization is due to an upgrade, that the copy of the application that resided on the original computer has been deleted and that no attempt will be made to reactivate it. I certify that the ISD software application(s) referenced above are used only in accordance with the licensing agreement under which it/they was/were purchased and that no illegal copies of the ISD software application are presently in use.
